Disconnecting a Facility from Omnivers

How practices and facility administrators end an existing EHR connection with Omnivers.

Overview

Disconnecting a facility from Omnivers takes two parties:

  • Your practice submits a deactivation request so we can track it and stop the data flow on our side.

  • Omnivers submits a request to the EHR vendor on your behalf.

Start here: submit your deactivation request
Done by the practice

Before the facility contacts the EHR vendor, submit a request so we know which connection is ending and can update your account. This also gives you visibility into the deactivation progress.

You must create a separate ticket for each facility you are disconnecting.

  1. In the chat bot, scroll down to Create a ticket.

  2. Select the Facility EHR Deactivation Request ticket type.

  3. Complete the required fields: Name of Facility, Facility EHR, and any others shown.

  4. Click Create ticket.

Once your ticket has been submitted, Omnivers will reach out to the vendor and submit a request on your behalf. Once the vendor completes the request, the facility will be removed from your facility list.
